Why Hormone Therapy Is So Underused
In 2002, the Women’s Health Initiative (WHI) study made headlines, suggesting hormone therapy was unsafe. What didn’t make headlines was the context:
- The women studied were, on average, 63 years old, long past menopause, and at higher risk of developing cardiovascular disease, blood clots, and osteoporosis.
- The therapy used synthetic progesterone and estrogen from the urine of pregnant horses. Far from the bioidentical ones used today. Bioidentical means the hormones have the exact structure as your own estrogen, progesterone, and testosterone.
- Results were misinterpreted, creating fear that has lasted decades and denied millions of women relief.

New research now shows a much different story: when started near menopause, or even in peri-menopause in some cases, and given in the right form, hormone therapy is safe, effective, and protective. Studies like KEEPS (2012), ELITE (2016), and WHI follow-ups confirm that early, bioidentical hormone therapy can:
- Reduce hot flashes and night sweats
- Improve sleep irritability and mood
- Support heart, brain, and bone health
- Lower long-term mortality risk
The Role of Estrogen
Estrogen supports nearly every system in the body, from your heart and brain to bones and skin. But how you take it matters:
- The preferred form is bioidentical 17β-estradiol, identical to what your body naturally makes.
- The safest way to take it is through the skin (patch, gel, or spray), which avoids the liver and reduces clot risk.
- Estrogen should never be taken alone if you still have your uterus; it must be balanced with progesterone to protect the uterine lining and reduce cancer risk.
Breast Cancer and Heart Health
Research from the WHI follow-ups found that:
- Women who took estrogen alone had lower breast cancer incidence and mortality.
- Those taking synthetic estrogen + progestin saw increased risk — not the same as bioidentical therapy.
- Starting estrogen early in menopause can improve cholesterol, enhance arterial health, and protect the heart.
